Downloadable PDF Forms

NEDIVSOLO2.ORG has a number of forms available in Adobe Acrobat® PDF format. In order to view these forms you must have Adobe Reader® (version 4.x or greater) installed on your computer. Adobe Reader is available to download for free from Adobe Systems Incorporated at ( click here ).

When you click on a download-form hyperlink, the form will either open in your current browser window, or you will be given the option of opening the file or saving it to disk. Choose either option. The advantage to saving the file to your computer is that you can print the form again later without having to download it again. If the form has opened in your browser and you wish to save it to your computer, simply select the save option.

Once you have opened the form, print it, fill it out legibly (unless it is a "fillable" form; if so, read below), and either mail it to the address specified on the form. Please DO NOT email the form to us.

When printing a form from within your Web browser do not use the browser's print function. Instead, use the print icon on the Adobe tool bar that appears immediately above the viewing field or the print button on the bottom of the form.

Several of the forms use very narrow margins. If you have problems with the edges of the document being "cut off" by your printer, select the "Shrink to fit" option in the Print dialog box. This option will proportionally reduce the document so that it will fit within the printable area for your printer.

If you encounter other printing problems, try downloading the latest version of Adobe Acrobat Reader®. If this does not help consult, your printer documentation or try Adobe's Customer Support Website.

Fill-in Forms Electronically - "PDF Fillable Forms"

For neatness and convenience, Adobe offers electronic fill-in forms. These electronic forms allow you to fill in the form on your computer before you print it off, producing a legible form that will assist the SCCA staff in processing your request. You are not required to fill the form out on your computer. You may print it and fill it out by hand if you prefer. Filling it in by hand may lengthen the time it takes the staff to process your request. In order to utilize the fill-in aspects of these forms you must have the latest version of Adobe Acrobat Reader®. To save an already completed form you must have the retail version of Adobe Acrobat®. If you do not have Adobe Acrobat, you can still fill out the form on your computer using Adobe Reader® and then print it out to mail. (Note: We will accept forms via email that do not require a signature; DO NOT send credit card numbers via email)

Once you have opened the form on your computer, select the hand-shaped tool from the Adobe tool bar. This tool allows you to move the form around on your screen. Position the hand tool over the first of the form fields and click. The I-beam cursor will then appear, allowing you to type in that field. For check boxes the hand will change to an arrow or a finger pointer allowing you to choose which option to check. Additionally, you may tab through the fields. Shift-tab allows you to go back a field, and the space bar will check a check box.

Once you have finished filling out the form, use your mouse to select an area of the form that is not inside a form field (for example, the white space in the margin around the edge) before you print. If a form field is active (the cursor is blinking within it) the contents will not print.
